Canada General Requirements

  • Duly accomplished draft application form – (You must answer every question. If not applicable, write N/A)
  • Duly accomplished form IMM 5476 (Use of Representative)
  • Original valid passport (6 months valid) and old passport with valid US visa if applicable
  • Original personal bank certificate (must include account type, current balance, account opening date, 6 months average daily balance).
  • Original bank statement or certified true copy of passbook for the last 6 months
  • Any additional relevant documentation (last 2 pay slips, property titles, proof of pension, investments, etc.)
  • Copy of recent personal income tax return
  • Hotel booking confirmation (if applicable)
  • Roundtrip airline ticket reservation (if applicable)
  • Detailed itinerary during your trip (if applicable)

Special Requirements

  • Original employment certificate (must include applicant’s position, date hired, compensation, office address, landline numbers and e-mail address)
  • Copy of PRC /IBP ID (if professional)
  • Copy of company ID
  • Copy of business registration from DTI or SEC
  • Copy of business permit and mayor’s permit
  • Copy of PSA marriage contract (If travelling with spouse)
  • Original school certification
  • Copy of school ID
  • Copy of PSA birth certificate (If travelling with immediate family)
  • Copy of parent’s documents, original bank certificate, original COE or business documents
  • Affidavit of consent from the non-accompanying parent(s) or legal guardian (if travelling with only one parent or without parents)
  • Copy of senior citizen card or retirement documents
  • Copy of PSA marriage contract (If travelling with spouse)
  • Copy of i-card
  • Original employment certificate for sponsorship
  • Copy of business registration from DTI or SEC
  • Copy of business permit and mayor’s permit
  • Additional requirements may be required by the embassy without prior notice.
