VISA FORMChina General Requirements
- Online visa application form (signed, must be printed on A4-sized paper). Kindly download and fill out the Chinese Visa Application Form and upload it through our Contact Us page or the in our form below.
- Two passport-sized color photos with a white background (taken within the last six months). Please avoid wearing white blouses or blazers.
- Original passport (must be valid for at least six months beyond the intended stay).
- Original old passport (if not available, provide an explanation letter).
- Copy of passport data page and emergency contact details.
- Original and copy of previously used Chinese visa (if applicable).
- Copy of roundtrip airline reservation.
- Copy of hotel reservation and daily itinerary.
- Original personal bank certificate (must have at least Php100,000 per applicant and include account type, current balance, account opening date, six months average daily balance, and attach the original official receipt/debit memo/certification of waived fee from the bank).
- Original bank statement or certified true copy of passbook for the last six months.
- Copy of recent personal income tax return.
- Copy of PSA birth certificate and/or marriage certificate (if traveling with immediate family).

- Original employment certificate with leave of absence (must include applicant’s position, date hired, compensation, office address and landline numbers and e-mail address)
- Copy of PRC/IBP ID (if professional)
- Copy of company ID
- Copy of business registration (DTI or SEC)
- Copy of business permit or mayor’s permit
- Copy of income tax return
- Original school certification
- Copy of school I.D.
- Copy of parent’s document such as original bank certificate, original COE or business documents
- Copy of senior citizen card or retirement documents
- Copy of i-card
- Copy of latest arrival stamp or E-Travel (if failed to provide, personal appearance is required)
- All documents must be issued within 1 month upon application.
- Additional documents may be required by the embassy without prior notice.
- Personal appearance is not required (unless the Embassy request for it).
